Yeovil Hospital, with our neighbouring Somerset NHS organisations and the County Council has published the county’s Sustainability and Transformation Plan (STP).
The plan sets out the challenges we are facing as a care system and describes why we need to change the way health and social care services are structured and delivered in order to meet the future needs of our local population.
These strategic, five-year plans are being produced across the country and aim to create services that work better for people and staff, are financially sustainable and have much greater emphasis on prevention of illness and injury.
Our STP sets out a vision for Somerset’s care system, rather than fully-developed proposals. Over the coming months there will be discussions with staff, patients, service users, carers and other interested groups to shape and develop these proposals for high quality, sustainable services.
