To book your first appointment with your midwife please complete the following form.

The form asks questions about yourself and your medical history.  Please answer as fully as possible, as this will ensure that you receive the most appropriate care.

Midwifery care is provided for women who live within our catchment area of East Somerset and North Dorset. If you live within our catchment area for community care but would like to have your baby at another maternity unit (e.g Taunton, Dorchester or Salisbury) please inform us of this choice when completing the self-referral form as we may need to complete additional paperwork. All booking appointments will be held at Yeovil Hospital, regardless of where you plan to have your baby.

Your referral will be processed by the maternity admin team and an appointment will be made for you between 8-10 weeks of pregnancy, you will then receive a text or email response from a member of our team with your appointment date and time – please allow 3 working days to receive your appointment details.

When attending your midwife booking appointment at the hospital you will be asked to bring a booking pack available from your GP Surgery, this contains your maternity handheld notes which will hold all the details about your pregnancy. Please collect this prior to your appointment as it is helpful if you have filled out as much of these as possible prior to attending.

The initial booking appointment can last between one to two hours depending on your previous obstetric and personal history, this is to ensure that midwives are able to plan and provide the best care for your pregnancy. Partners and family are welcome to attend the appointment, we will be able to discuss with you any questions or concerns about pregnancy, book scans, screening tests, and outline when and where you need to attend appointments in your pregnancy.
